We are excited to announce a new social work program at the High Risk Pregnancy Center, This program is designed to provide compassionate social work services and essential resources to patients facing complex pregnancy situations. Alexandra offers comprehensive support to help clients adjust to life changes and challenges such as unemployment, housing displacement, transportation issues. Alexandra helps patients navigate through community services by determining what type of aid they need. She will refer patients to community resources and establish rapport with other agencies to facilitate additional resources.

Comprehensive Social Work Support for High-Risk Pregnancies
Our High Risk Pregnancy Center is committed to providing compassionate social work services and essential resources to patients facing complex pregnancy situations. Our social worker will assess needs, advocate, and case manage and intervene in crisis situations while ensuring both physical and emotional well-being during and after pregnancy.
Intake and Needs Assessment
Our social worker begins by assessing each patient’s needs through a detailed intake process. This includes questions to identify social determinants that impact health, such as housing insecurity, transportation, access to supplies, and potential safety concerns. Our social worker will help protect and promote patient’s wellbeing so that they can enjoy better quality of life.
Community Resource Navigation
Our social worker specializes in connecting patients to vital community resources. These resources include access to:
- Domestic violence shelters
- Low-income housing assistance
- Baby supplies and diaper banks
- GED assistance programs
- Food banks and nutrition support
- Discounted or free transportation (RTC for medical needs)
Follow-Up and Ongoing Support
Depending on the patient’s needs, our social worker provides follow-up support to ensure connections to community services are effective. She stays in touch with patients through scheduled appointments, phone calls, or in-office visits, offering guidance and tracking progress.
Grief Counseling and Trauma Support
For mothers experiencing grief or trauma, Our social worker offers individualized support and facilitates connections to support groups. We believe that healing is enhanced through shared experiences, and we encourage participation in support groups for those experiencing pregnancy loss or navigating difficult situations.
Collaboration with Medical Providers
Our social worker collaborates closely with OBs and other healthcare providers to ensure seamless care. Patients can be referred directly through their provider, and our team works together to develop a comprehensive care plan addressing both medical and psychosocial needs.
Accessible Multilingual Care
Our social worker is fluent in both Spanish and French, allowing them to connect with and support diverse patient populations. We are committed to making our services accessible to all.
Innovative Resource Access
We’re making community resources more accessible through initiatives like QR codes linked to comprehensive lists of local services. Patients can easily scan the code to access the information they need right from their phone.
